Pre-Survey Questionnaire

Document review and interviews are the second component of the assessment, and the pre-survey questionnaire is how they start. The guide asks the consultant to send the owner's point of contact a questionnaire before the walkthrough and to interview that person during it; together they cover ownership and management history, the ages of major systems, historical repairs and their costs, warranties, preventive maintenance, pending or planned work, code citations, litigation about physical condition, hazardous-materials surveys, accessibility complaints, flooding, and tenant-owned equipment.

Every answer is "reported" information in the report's attribution scheme, credited to the person who gave it, and the questionnaire itself is included as an exhibit. See the document request list.
